2012 Application Instructions: The application window for the class beginning in July, 2012 will open on 1/18/2012 and close on 2/18/2012.  Applications received after 2/18/2012 will not be considered.  If you plan to hand-deliver your application, you must do so by close of business at 4:30 pm on 2/17/2012.  Human Resources will not be open to accept hand-deliveries on Saturday, 2/18/2012. 

Information contained on or attached to this application must be true, correct, complete and made in good faith.  Fraudulent or false information are grounds for exclusing from consideration or dismissal from the program. 

1. Resume
- Submit a resume detailing your a) full legal name, address, phone and email, b) colleges and universities attended including degree(s) earned, and c) employment experience for the past 5 years (minimum) including the employer's name, address and phone.  Note that each page of your resume should be numbered AND contain the position title (Nuclear Medicine Technologist Trainee) plus announcement  number (12-011LF).  

2. OF-306 - Complete the Declaration of Federal Employment.  This can be downloaded here.  While this document was designed for employees, prospective students must also complete and sign.

3. Transcripts
: Applicants should include a copy (unofficial) of their college transcripts. Please highlight coursework in: Chemistry, Physics, Math, Oral Communications, Written Communications and Anatomy/Physiology; and,

4. Letter of Intent: Include with your application a brief (250 word or less) essay describing why you want to become a nuclear medicine technologist; and,

5. Related Experience: Include with your application a brief (150 word or less) description of relevant knowledge, skills or abilities in either the health care or nuclear industries; and,

6. Current Licensure: If you're licensed in another allied health field, please include a copy of your certificate. Note that this is not a requirement for acceptance.

7. Preferred Training Site: Please indicate your preferred training base: Palo Alto/Bay Area, Sacramento and/or Fresno.  Note that we cannot always match students with their regional preference. Students who do not indicate their preferred training site will be considered for all locations.

8. Recommendations: Include contact information (phone and email) for your referrals. The referrals should be from supervisors, instructors or other superiors with first-hand knowledge of your skills/abilities and work ethic.  You may also include letters of recommendation with your application, but these are not required.
